MSHSAA PHYSICAL FORMS ARE GOOD FOR 2 YEARS IF DOCTOR APPROVED!
The Liberty Athletic Department uses Final Forms for all required documentation, physicals, emergency contact information. No paper copies of physicals are accepted.
All accounts must be updated with a current sports physical prior to the first day of practice.
Parents can still access Privit to download previous physicals to import to the new system. Privit Link.
