MSHSAA PHYSICAL FORMS ARE GOOD FOR 2 YEARS IF DOCTOR APPROVED!
The Liberty Athletic Department uses Final Forms for all required documentation, physicals, emergency contact information. No paper copies of physicals are accepted.
All accounts must be updated with a current sports physical prior to the first day of practice.
